Draft for review — not yet legally binding. This document is a starting point written to be edited. It has not been reviewed by an attorney. Before publishing, have a licensed attorney in your state review it — particularly the sections on health information, scope of practice, cancellation and refunds, and limitation of liability. Anything in [square brackets] needs a real value filled in.

Information we collect
Information you give us directly
- Contact details — your name, email address and phone number when you request a consultation or send a message through the website.
- Consultation details — the category you select (for example, recently diagnosed, in treatment, survivor, caregiver), your preferred contact time, and anything you choose to write in the message field.
- Client records — if you become a coaching client, the intake questionnaire, session notes, goals, wellness plans, progress tracking, and correspondence relating to your coaching.
- Payment information — billing details needed to process payment. Card numbers are handled by our payment processor, not stored by us. [CONFIRM PROCESSOR — e.g. Stripe, Square, PayPal.]
Information collected automatically
- Standard server and analytics data such as IP address, browser type, device type, pages viewed, and referring site. [CONFIRM WHICH ANALYTICS TOOL, IF ANY, IS IN USE.]
Please keep website messages general. The contact form is not a secure channel. Do not send diagnoses, test results, treatment details or other sensitive medical information through it. We will arrange a private conversation instead.
How we use your information
We use the information described above to:
- respond to your enquiry and schedule your complimentary consultation;
- determine whether coaching is an appropriate fit for what you are seeking;
- provide coaching services, including preparing for and following up on sessions;
- send session reminders, resources, workbooks and administrative messages;
- process payments and keep business and tax records;
- improve the website and our services;
- comply with legal obligations.
We do not sell your personal information, and we do not share it with third parties for their own marketing purposes.
If we send occasional newsletters or updates, you may opt out at any time using the unsubscribe link or by emailing us. Opting out of marketing does not stop administrative messages about coaching you have booked.
Health information and HIPAA
Hope & Strength Coaching is not a HIPAA-covered entity. Coaching is not a licensed healthcare service, so the information you share with us is generally not protected health information under HIPAA. We nonetheless treat what you tell us as confidential and handle it with care, as described in this policy and in the Coaching Agreement.
If you would like information shared with a member of your medical team, we will ask for your written authorisation first. [CONFIRM WITH COUNSEL WHETHER A WRITTEN RELEASE FORM SHOULD BE ADDED HERE.]
When we share information
We share personal information only in these situations:
- Service providers who help us operate — scheduling, video conferencing, email delivery, payment processing, and file storage. They may access information only to perform work for us. [LIST ACTUAL VENDORS.]
- With your permission — for example, if you ask us to coordinate with a family member or a member of your care team.
- Safety — if we reasonably believe disclosure is necessary to prevent serious harm to you or another person.
- Legal requirement — to comply with a subpoena, court order, or other legal obligation.
- Business transfer — if the practice is sold or transferred, subject to this policy.
How long we keep information
We keep client records for as long as needed to provide services and to meet legal, tax and accounting requirements — generally [ADD RETENTION PERIOD, e.g. seven years] after your last session. Enquiries that do not become coaching relationships are deleted after [ADD PERIOD]. You may ask us to delete your information sooner; see below.
Your privacy rights
Depending on where you live, you may have the right to:
- request a copy of the personal information we hold about you;
- ask us to correct information that is inaccurate;
- ask us to delete your information;
- opt out of marketing communications;
- ask us not to sell or share your information — note that we do not sell personal information.
California residents
California residents have rights under the California Consumer Privacy Act (CCPA/CPRA), including the rights to know, delete, correct and opt out, and the right not to be discriminated against for exercising them. To make a request, contact us using the details below. We will verify your identity before acting on a request.
[HAVE COUNSEL CONFIRM WHETHER CCPA/CPRA APPLIES TO THIS BUSINESS BASED ON REVENUE AND DATA VOLUME THRESHOLDS, AND WHETHER ADDITIONAL DISCLOSURES ARE REQUIRED.]
How we protect information
We use reasonable administrative and technical safeguards to protect your information, including access controls, reputable service providers, and limiting who can see client records. No method of transmission or storage is completely secure, and we cannot guarantee absolute security. Email and web forms are inherently less secure than a private conversation.
Cookies and website analytics
This website may use cookies or similar technologies to keep the site working properly and to understand how visitors use it. You can control cookies through your browser settings; blocking them may affect how parts of the site function.
[CONFIRM WHETHER ANALYTICS, ADVERTISING PIXELS OR CHAT TOOLS ARE INSTALLED. IF ADVERTISING PIXELS ARE ADDED LATER, THIS SECTION AND THE CCPA DISCLOSURES MUST BE UPDATED.]
Children's privacy
Our services are intended for adults. We do not knowingly collect personal information from anyone under 18. If you believe a minor has provided us information, please contact us and we will delete it.
Changes to this policy
We may update this policy from time to time. The date at the top of this page shows when it was last revised. Material changes will be highlighted on the website.
